Privacy Policy

As part of the occupational therapy services provided, the confidentiality of your personal information is essential. All data collected during interventions is protected and used solely to deliver high-quality professional support, in accordance with Canadian privacy standards, including Quebec’s Law 25 on data protection.

Collection of Information

The following information may be collected through web forms, email, phone, or during sessions:

  • First and last name, address, email, phone number

  • Clinical information necessary for the intervention (reason for consultation, history, current situation)

  • Availability and contact details of an authorized individual, if applicable

  • Billing information (e.g., bank transfer details, if applicable)

 

Only the information necessary for assessment, intervention, and communication is retained.

 

Storage of Records

Clinical information is securely stored in an electronic file (Microsoft 365 – OneDrive), hosted in Canada (Microsoft data centers). This system complies with security standards (encryption, restricted access, authentication). There is a low risk that some data may be subject to foreign laws; however, all reasonable efforts are made to minimize this risk.

 

Retention and Destruction of Records

In accordance with the regulations of the Ordre des ergothérapeutes du Québec (OEQ), your file will be kept for five (5) years following the end of services, after which it will be permanently deleted. Any paper documents are shredded once scanned or deemed non-essential.

Virtual Consultations

Remote sessions are conducted via Microsoft Teams, with security measures such as a virtual waiting room and controlled access. Despite these precautions, zero risk cannot be guaranteed. It is recommended to use a personal device in a quiet, private setting to protect your confidentiality. By using remote services, you agree to:

  • Provide accurate information about your location (province), to ensure compliance with practice regulations in Quebec

  • Inform your occupational therapist of any changes to your contact information

  • Acknowledge that despite best efforts, technical limitations outside of our control may affect confidentiality.

Email Communication

Email may be used for scheduling, follow-ups, or document sharing (e.g., reports, intervention plans). Files sent via email are password-protected to reduce risks. However, please be aware that electronic communications carry inherent security limitations.

Consent

By using our services (forms, scheduling, email contact), you consent to the collection and use of your information as described in this policy.

Disclosure to Third Parties

Information may only be shared with another professional or organization (e.g., insurer, physician, other service provider) with your written consent. No reports will be shared without your prior authorization.

Professional Confidentiality

As occupational therapists, we are bound by professional confidentiality. Your information will only be shared with your consent, or if legally required (e.g., court order, imminent risk to yourself or others).

Your Rights

You may request access to your file or ask for corrections at any time. In the event of a privacy incident, legal obligations will be followed and you will be notified accordingly.
